Scientific Seminar and Technical Visit to King Chulalongkorn Memorial Hospital

King Chulalongkorn Memorial Hospital (KCMH) is one of the leading university hospitals in Thailand. KCMH is the first hospital in Asia to use MRI simulation. It is also the first hospital and regional training centre in Thailand to employ MRI-based image-guided brachytherapy for gynecological cancer. This event provides a good opportunity for scientific exchange between members of KCMH and HKIPM.

Date: Saturday 22nd November - Monday 24th November, 2014 (3 Days & 2 Nights)

Programme:

22 Nov.: a.m. Hong Kong to Bangkok, hotel check-in & free schedule

23 Nov.: a.m. (1) Scientific seminar (see following page for details of the programme)

(2) Visit radiotherapy facilities of KCMH

p.m. Free schedule

24 Nov.: a.m. Free schedule

p.m. Hotel check-out for airport to Hong Kong

Cost: Around HKD 4000* for member

Around HKD 4500* for accompanying person

*The exact amount will be confirmed upon confirmation of air ticket reservation.

*The cost includes round trip air tickets, hotel accommodation on a double occupancy basis (at own extra cost for room of single occupancy) and local transportation between Bangkok international airport and hotel. The cost does not include individual travel or any insurance.

Accompanying Person:

A maximum of one accompanying person per member is allowed. The accompanying person may not join the technical/scientific programme on 23 Nov. They may have their own programme.

2 Please register with the full name and gender of the member and accompanying person (if any) for air ticket and hotel reservation.

3 Applicants should indicate single or double hotel room occupancy at the time of registration. In the latter case, an applicant may indicate the name of the other person with whom he/she wish to share a room with.

4 A cheque made payable to “Hong Kong Institution of Physicists in Medicine” for the above amount shall be submitted at the time of registration application for deposit. Once registration is confirmed by HKIPM, the deposit shall be non-refundable.
